Distinctions Salon Is Working In And With Our Communities !!!

We at Distinctions Salon are excited about forming partnerships with various Community Projects and providing COVID19 Relief  because we care and want to provide support. We also want to show our appreciation to the community for the support we have received. Distinctions sets aside a small budget quarterly for us to reach out into the community through charitable works.  We typically provide support on a first come, first serve basis, but all decisions are decided by management.

Instructions

INSTRUCTIONS FOR SUBMITTING YOUR COVID19 ASSISTANCE REQUEST FORM:

  1. You Must Register with Distinctions Salon to Access the COVID19 Assistance Request Form.
  2. When submitting your application, you must attach, with your application, a short video telling us about the difficulty you are having and any supporting documentation of the difficulty you are experiencing.
  3. Registered submissions will be evaluated by our board of staff members and selection for approval will result from majority vote.
  4. You must complete and sign the “Consent for Release” form, which we will send you upon approval of your application and return by email with a photocopy of your State of Arizona issued Driver’s License.
  5. You must be at least 21 years of age to apply for assistance.
  6. Only one submission per household.
  7. Distinctions Salon reserves the right to make the final decision on approval and the amount of the assistance.
Qualifications:
  1. We will accept all requests for assistance However, 1st priority will be given to our clientele, 2nd to Community Members within our zip code, and 3rd Community Members who live within a 10 mile radius of the salon location and 4th all other requests.
  2. You must be at least 21 years of age to apply for assistance.
  3. You must register at distinctionssalon.com.
  4. You must complete the COVID19 Assistance Request Form.
  5. You must complete and sign the “Consent for Release” form, which we will send you upon approval of your application and return by email with a photocopy of your State of Arizona issued Driver’s License.
  6. You cannot have received prior assistance under this initiative from Distinctions Salon.
  7. You cannot be a family member of any Distinctions Salon staff member.

INSTRUCTIONS FOR SUBMITTING YOUR COMMUNITY SERVICE REQUEST:

  1. You Must Register with Distinctions Salon to Access the Community Service Request Form.
  2. All requests must complete and submit the Community Service Request Form.
  3. Please Complete the “Consent for Release Form” prior to funding.
Qualifications:
  1. Your request must be submitted by a representative of your organization who is at least 18 years of age.
  2. The event must benefit our identified community because we do not accept requests for individual SOLICITATIONS.
  3. Your organizations must have tax ID.
  4. Only one submission per quarter is allowed.
  5. Events and Organizations should be located within 10 miles of our location at 2240 N. Scottsdale Road, Tempe, AZ 85281.
  6. All requests must be submitted at least 60 days prior to the event.
